MEMBERSHIP PLAN: TERMS & CONDITIONS
- Services included with the dental plan can not be redeemed against a new patient examination or first hygiene visit.
- Six installments of the monthly plan fee will be required prior to attending for dental appointments or hygiene visits included with the dental Preventative Plan. For patients joining the Hygiene Care Plan, we require three monthly payments before you attend your first Hygiene appointment as part of your plan.
- Depending on your membership plan, you will be entitled to appointments with us every 3 or 6 months. While we send reminder emails, it’s the patient’s responsibility to ensure that they book their appointment at the time that they are due. Your appointments will then be available on a rolling 6-month basis for Preventative Plan Patients, and on a rolling 3-month basis for Hygiene Care Plan patients.
- All plan patients will receive a 10% treatment discount off routine treatments.
- There is a 12-month minimum term for the dental plan. In the event that a member ceases their membership contribution within the first 12 months, the Practice reserves the right to reclaim the difference in financial value of any benefits received by the member. Such a calculation will be based on a comparison of benefits received when compared with the private fee scale of the practice. Following the initial 12-month membership period, 1 month’s notice is required to terminate membership.
- There will be a one-off administration fee of £8.00 payable with the first direct debit payment.
- Dental examinations and Hygiene services included on the dental plan must be undertaken within 4 weeks of each other, however we do recommend that they are completed on the same day.
- Adult dental plans include 2 dental examinations in a 12-month period. One of the dental examinations will be completed by one of our Dental Therapists and will include an bespoke health report which details any areas of concern, and areas for you focus on before your next visit.
- Payment for the dental plan will be taken by direct debit on the 1st of every month. We are unable to change this date. The monthly fee paid by direct debit may be increased by the Dentist providing one month’s notice in writing to the patient.
- If for any reason the monthly direct debit fails on more than one occasion, the remaining fee for the rest of the plan term is required to reinstate the dental plan and to use the services and benefits associated with it. In this instance, if the plan is not reinstated the practice reserves the right to reclaim the difference in financial value of any benefits received by the member.
- The plan agreement is not transferable between the patient and other patients, and only covers the services available with the Dentist and their nominated deputy in his/her absence.
- The treatment discount associated with your plan cannot be used in conjunction with promotions or any treatment carried out by specialist dentists who are registered with the GDC as a specialist. The Patient agrees to pay fees to The Dentist for additional dental treatment completed that is not included in this plan.
- It is the patients’ responsibility to read the supplementary Global Assistance Scheme cover provided under the dental plan to ensure any potential claim will be covered. The practice takes no responsibility should a claim be disallowed due to the clauses that are outlined in the policy document.
